A Family Support Coordinator works with families who have a child/youth with a developmental disability. We assist by supporting and empowering families, with their child's needs in mind.
Our Family Support Coordinator can Offer:
- Information on Services, Programs & Opportunities
- Referrals/liaison with other agencies, schools
- Individual program planning
- Assistance with various Government Forms
- Attend school, medical and assessment conferences with the family
What is our Purpose?
To support families in caring for their children with disabilities and/or special needs.
Who is Eligible?
Families who live in southern Oxford County and have a child under 18 years of age (with intellectual disabilities and /or developmental delays).
Who Can Make a Referral?
The family, or any other source, with the families’ consent. Is it Confidential? Information relating to the required services is obtained and only shared with specific agencies or professionals with the consent of the parent.
How are Families Supported?
Family Support Services help families identify their needs and assist in accessing available services. Parents are encourages to be their own advocates and service coordinators, whenever possible. Pertinent information is provided in a timely manner.
Is There a Cost?
This Service is funded by the Ontario Ministry of Community and Social Services. For eligible families, there are no fees for using our services. This FREE service is available for families in south Oxford County and the Tillsonburg Tri-County Community. Parent referrals are welcome.
